Map Your Care was formed in 2021 to help change the culture of how illness, dying and grief are talked about in our society. Coping with these parts of life is challenging enough–we don’t need the additional stigma of a taboo approach! With over 12 years of experience in serious illness care, we weave our experience in a variety of medical settings into individual, couples and group therapy to support tough conversations, helping you to live a full and meaningful life at any health.
We utilize traditional talk therapy, including Cognitive Behavioral Therapy (CBT) and Acceptance and Commitment Therapy (ACT) to help you align with your values as you navigate some of life’s most difficult challenges. With backgrounds in hospice, palliative care, rehab, dementia care and oncology, we bring a strong understanding of the complexities of healthcare and the connection between physical and mental health.
Our Serious Illness Group and Navigating Caregiving Group start January 2025 and will be offered again later in the year.
We also offer in-home sessions for clients who have difficulty getting out of the home.
